When NewLeaf opens, patients will complete a simple intake process. To request an appointment or ask questions before we open, please contact us.
What to expect at intake:
Completion of patient information form (name, contact, insurance provider)
Review of services, costs, hours, and available policies
Explanation of recipient rights and complaint process
Substance use history assessment
Consent for treatment discussion
Naloxone access education and offer
Service plan development with licensed professional
All patient records will be confidential and secured per federal and state regulations (42 CFR Part 2).

Care will be delivered by Michigan-licensed prescribers and counselors — our clinical team is being finalized ahead of opening.

NewLeaf will serve patients from surrounding communities including Haslett, Okemos, Holt, East Lansing, DeWitt, Grand Ledge, Charlotte, and St. Johns. Our location at 400 S. Walnut Street is easy to reach by bus from the CATA Transportation Center.

Yes. All patient records will be confidential and secured per federal and state regulations, including 42 CFR Part 2 and Michigan R 325.1369. Disclosures require legal permission, rule requirement, or written patient release.
